SAMPSON COUNTY HEALTH DEPARTMENT

SampsonRMC and Sampson County Health Department partner together to provide the community with the most effective and efficient diabetes education and support resources.


CHRONIC DISEASE MANAGEMENT

According to a study completed by John Hopkins University, more than 145 million people nationwide are living with a chronic disease.* Sampson Regional Medical Center is committed to improving the health and wellness of our community by providing education and support to help prevent or minimize the effects of a disease.


DIABETES EDUCATION

SampsonRMC and Sampson County Health Department partner together to provide the community with the most effective and efficient diabetes education and support resources. 

Sampson County Health Department oversees the Diabetes Self-Management Education Program, which is designed to help you better understand how to control your disease and live well with diabetes.  

Learn more about:

  • Maintaining overall health
  • Meal planning and nutrition
  • Monitoring blood glucose
  • Physical activity
  • Reducing risk of complications
  • Setting goals & tracking progress

A referral from your primary care provider is needed for participation.  An education record for you will be maintained and a summary will be sent to your primary care provider so they can monitor your progress as it relates to your overall health.
